H. Pylori: Oh, The Sneaky Trouble It Can Cause

H. pylori may be a tiny bacteria living in the stomach, but I consider it a pretty big deal when it comes to overall health. Why? Because it can interfere with one of the most foundational processes in the body—our ability to properly digest food and pull the nutrients from it.
Think about digestion from the top down. Before your body can use iron to build healthy blood, amino acids to make neurotransmitters, or vitamins and minerals to support energy, hormones, immunity, and literally thousands of biochemical processes, you first have to be able to digest and absorb them.
And this is where H. pylori can quietly create trouble. It can disrupt the stomach environment, interfere with digestion and nutrient absorption, and create ripple effects much farther downstream.
Sometimes those clues are obvious—reflux, burning, nausea, stomach pain, bloating, or burping.
But sometimes they look nothing like a stomach problem at all: stubborn low ferritin, nutrient deficiencies, SIBO, food reactivity, histamine issues, or immune dysfunction.
And sometimes? There are almost no digestive symptoms. That is what makes H. pylori such a sneaky little troublemaker—and one I don't like to overlook.

So, What Exactly Is H. Pylori?
Helicobacter pylori (H. pylori) is a spiral-shaped bacteria that can live within the protective lining of the stomach. H. pylori has actually lived alongside humans for thousands of years, and not everyone who carries it develops symptoms or disease. Like so many things in the microbiome, the bigger question is what happens when the relationship becomes problematic.
When H. pylori begins disrupting the stomach environment and driving chronic inflammation, that's when this little bug can start making trouble. And it has some pretty clever survival skills.
H. pylori can change the stomach environment—and stomach acid can shift over time. Earlier patterns may be associated with increased acid production, contributing to reflux, burning, or indigestion. For some people, that can lead to acid-suppressing medications such as PPIs without ever identifying why those symptoms started.
Over time, chronic H. pylori gastritis can contribute to lower stomach acid, creating a different set of problems involving digestion, nutrient absorption, and microbial balance.
And H. pylori can hang around for years—or even decades—without you realizing it is there.
Sneaky? I think so.
How Did This Little Sneaker Get Here?
H. pylori is contagious and can spread from person to person through oral-oral and fecal-oral transmission. Contaminated food and water can also be sources.
But one of the important things to know is that H. pylori commonly clusters within families. It is frequently acquired during childhood, which means when one family member tests positive—especially when others have suspicious symptoms—it may be worth considering the bigger family picture.
And yes, kids can have H. pylori too. Children may carry it without obvious digestive symptoms, which is another reason I don't think we should assume “no stomach pain” means everything upstream is working perfectly.
The Really Tricky Part? You May Not Know You Have It.
Some people have the obvious symptoms: Reflux. Burning. Stomach pain. Nausea. Bloating. Burping. Feeling overly full.
But others have very few digestive symptoms at all. Instead, the clues may start showing up farther downstream—stubborn low ferritin, nutrient deficiencies, recurring SIBO, increasing food reactions, histamine issues, or immune dysfunction.
And this is where H. pylori gets really sneaky. What looks like several unrelated health challenges may actually have an important piece of the puzzle sitting much farther upstream—in the stomach.
So, let's connect some of those dots.

Oh, the Sneaky Trouble It Can Cause
Digestion + Nutrient Absorption
Your stomach is one of the first major steps in digestion, and stomach acid is there for a reason. It helps break down protein, release nutrients from food, and protect the digestive tract from unwanted microbes.
When H. pylori disrupts this environment—particularly when stomach acid becomes chronically low—digestion and nutrient absorption can suffer downstream.
Iron + That Ferritin That Just Won't Budge
This is a BIG one for me. If someone has chronically low iron or ferritin despite eating iron-rich foods or taking iron supplements, I want to know why.
H. pylori can interfere with iron in several ways. Lower stomach acid can make dietary iron harder to absorb, chronic infection can affect how the body handles iron, and H. pylori itself needs iron to survive.
So sometimes stubborn low ferritin isn't simply an “I need more iron” problem.
There may be a little troublemaker sitting upstream.
B12 + Other Nutrients
Iron isn't the only nutrient that can take a hit. B12 absorption depends heavily on what happens in the stomach, and chronic H. pylori gastritis can interfere with the environment needed for normal B12 absorption.
And when digestion isn't functioning well overall, I want to think beyond one nutrient and look at the bigger nutritional picture.
SIBO, Leaky Gut + Food Reactivity
Remember, lower stomach acid means food may not be broken down as efficiently, leaving larger, incompletely digested food components moving downstream. At the same time, reduced stomach acid means fewer unwanted bacteria are killed before reaching the small intestine—one way H. pylori may help set the stage for SIBO and dysbiosis.
That disruption can continue farther down the GI tract. Poor digestion, bacterial overgrowth, and inflammation can compromise the intestinal barrier—or “leaky gut.” When the barrier becomes more permeable, food components and bacterial products can have greater contact with the immune system, potentially triggering increased food reactivity and systemic inflammation.
And now we have moved from one little troublemaker in the stomach to poor digestion → bacterial imbalance → leaky gut → food reactions + an increasingly activated immune system.
Which brings us right into histamine and autoimmunity.
Histamine + Mast Cells
Now our little troublemaker is getting farther away from symptoms you would automatically connect to the stomach. H. pylori creates an immune response in the stomach and can interact with mast cells and histamine signaling.
That doesn't mean H. pylori automatically equals histamine intolerance. But when H. pylori shows up alongside migraines, flushing, itching, food reactions, nasal congestion, heart racing, or other histamine-type symptoms, it becomes another potential contributor to the histamine bucket.
Instead of only asking how to lower histamine, I also want to ask: What keeps filling the bucket?
Immune Function + Autoimmunity
H. pylori also creates ongoing interaction with the immune system and has been studied in connection with several immune and autoimmune conditions. That does not mean H. pylori causes autoimmunity.
But when someone is already dealing with immune dysregulation, chronic infections are one of the pieces I want to investigate. Sometimes our little troublemaker is simply one more thing keeping the immune system busy.

So...How Do We Know If H. Pylori Is There?
There are several ways to test for H. pylori, and each has limitations.
A urea breath test looks for evidence of an active infection, but H. pylori can sometimes be missed—especially when medications such as PPIs, antibiotics, or bismuth suppress bacterial activity.
Endoscopy with biopsy allows the stomach tissue to be tested directly, but it is invasive and can potentially miss H. pylori because only small areas of the stomach are sampled.
And then there is my favorite - a stool test. This is a simple, non-invasive option that can even be used with our little ones.
In my practice, I use the GI-MAP because I don't just want to know if our sneaky little troublemaker is there. I want to understand what else is happening in the gut around it.
